Which Scantron forms can I use with the ParScore scanner?
You will learn which Scantron forms are acceptable to use with the ParScore scanner in the Faculty Commons (PLS-237) at Cal State Fullerton.
In order to use the computerized ParScore scanner, you will need to use one of the acceptable Scantron forms listed below.
Scantron Form F-1712-PAR-L
This is the most popular form used on campus. This form is recommended as the best choice for the first exam of the semester, as it will enable the auto-enrollment of students into ParScore.
Form description:
- Red, 8.5” x 11”, 2-sided
- Student enrollment (auto enroll)
- 200 questions with 5 response choices (A-E) divided into 4 sections
- Backside has lined space for manually-graded essay response
- Price per form at TitanShops: $0.32
Scantron Form F-289-PAR-L
Use this form only after having used an auto enroll (self-enrollment) form, like the F-1712-PAR-L form mentioned above because this F-289 form does not auto enroll students into ParScore.

- Red, 4.25” x 11”, 2-sided
- Price per form at TitanShops: $0.32
- Side 1: 100 questions with 5 response choices (A-E) (questions 1-100)
- Side 2: 100 questions with 5 response choices (A-E) (questions 101-200)
Scantron Form X-101864-PAR-L
Use this form only after having used an auto enroll (self-enrollment) form, like the F-1712-PAR-L form mentioned above because this F-289 form does not auto enroll students into ParScore.

- Red, 4.25” x 11”, 2-sided

- Side 1: 50 questions with 5 response choices (A-E) (questions 1-50)
- Side 2: 50 questions with 5 response choices (A-E) (questions 51-100)
Scantron Form F-288-PAR-L
This form can be used as the first exam of the semester, because students will be auto-enrolled in ParScore. However, one drawback of this form is that the students must enter their ID number on BOTH sides of the form. Otherwise, the form will be rejected.

- Red, 6” x 11”, 2-sided
- Side 1: Student enrollment
- Side 2: 200 questions with 5 response choices (A-E)
- Price per form at TitanShops: $0.32
Note
Do not accept torn, worn out, or wrinkled forms from your students as these will cause a paper jam in the scanner. Students should use only use a #2 PENCIL on the form. The student’s score will NOT be printed on any of the Scantron forms by the Scantron machine. Use the ParScore software to save, print, and/or export the scores as an Excel file which can then be uploaded to your gradebook in Canvas, if desired.
